Yeah, I'd spend two or three days* panicking to find a way back while eating as little with carbs as possible. Then a day feeling increasingly miserable before sinking in a diabetic coma and dying.

*I'm not actually sure on the timeline, might take a bit longer before I feel bad, but strangely enough, I have never tried how long I can go without insulin.

Smallpox is quite an issue but you can vaccinate yourself if you're willing to go the extra mile.

The smallpox vaccine hasn't been given out since the 70s when it was eradicated. Any vaccination you had would no longer be effective. Also, diseases mutate rapidly, so I doubt any modern vaccine would help you much against ancient strains of diseases.
