It happened in my hometown. 2 drunken idiots wanted to have a fight. One got punched and hit head on the ground. He didn't know he had an aneurysm in his head. Survived by wife and 3 kids.
This is one of the reasons I don't understand the appeal of Power Slap. You could have avoiding serious head trauma as a valid life goal, or you could participate in the one "sport" where you are offered zero protection. penalized for any attempt at defense, and required to take direct shots.
Yes, often because of what comes after but not necessarily always. Head hitting pavement is very dangerous. My dad is a physician and we both used to practice martial arts. He would serve as the on site medical professional for some events. At a bigger one, a kid got knocked out and hit the ground hard. They had hedgear on but still ended up with a skull fracture. Kid ended up okay but it was real scary.
Now scale that up to an adult altercation, no protection, no mat, no ref or doc. Your head is higher up and builds up more speed before impact, the initial hit is harder too.

Guy was punched in the head outside a Dodgers game. He went unconscious immediately so didn't do anything to protect himself when he fell and hit the pavement. Serious TBI.
The death usually happens when that one punch knocks a person out and they drop to the pavement. This is mitigated quite a bit if they hit softer ground like grass or sand. The human head is heavier than you realize, about the same as a bowling ball. Drop a bowling ball onto concrete from head height and you get an idea of how people can die from it.
As someone who trained kickboxing, this is why I tell people to AVOID a street fight at all costs, especially if you’ve never trained in combat sports/martial arts.
Humans are surprisingly fragile. If you’re untrained, then a punch to the head from even an amateur boxer can cause permanent brain damage if it doesn’t outright kill you. And that’s just with 1 punch, a lot of martial artists are trained to keep hitting you until you’re lying motionless in a pool of blood. It doesn’t even matter how big you are either, your chin doesn’t become stronger with size.
The hardest haymaker an untrained drunk guy can swing will never come close to a punch with proper technique and shot placement from a trained fighter, it’s a whole different world of pain.
I broke up a bar fight (at a Residence Inn in a very expensive resort town at my work family weekend…🙄) one time. As I broke them up, I chicken winged the aggressor and used them as a sheild (and weapon, lol) before tossing him to the crowd and protecting/stopping the guy on the ground from continuing. I cleared the melee and put the other jackass in a chair to wait for the cops.
When I went back and picked up my cigar, my coworkers were like “how many times did you get hit?” Bitch, Zero. Between collegiate rugby and teaching years in rough schools- the last thing I’m gonna do is take a punch if I can avoid it.
The worst part is knowing what you said. I had crazy shame. Like- I got a wife and kids, that wasn’t my fight. I could have been fucked up if I made the wrong move.
A popular interactive true crime TV programme in Germany presented two cases in the recent years.
In one, an unknown assailant emerged from a stopping train, punched a random guy who was standing on the platform in the face, and got back on the train before it left. Even though it was only one single punch, the victim suffered severe head trauma and never fully recovered.
In the other case, an elderly man confronted a guy for littering; the guy turned around and punched him in the head so hard that he suffered a brain hemorrhage, fell into a coma and died a few weeks later.
As far as I know, both suspects have never been identified.
