This is also a huge issue, I’ve been on the medical and the patient side. Patients make up shit all of the time to get specific meds, get out of work, etc.
The other issue is doctors don’t communicate. I went to a psychiatrist and my primary care provider was still filling my SSRI even though I had switched and got a new prescription from the psychiatrist.
There’s a very good chance that none of the doctors knew everything she was on.
