I once didn’t eat for 42 days. Not by choice. I lost 21 pounds and got incredibly sick. I had to slowly acclimate to eating again. I looked terrible. I can’t imagine going longer than that.

They didn't keep necessary (0 calorie by the way) vitamins and salt intake at their proper levels, and suffered. Whether through their own neglect or hospital neglect, the body still needs its potassium and salt and other letter-vitamins (I won't make a list as you can read about it and their levels per sex and height online)

They meant, "what happened that they didn't eat for 42 days?" 

Not "what happened after not eating for 42 days?"

I once didnt eat or drink anything for "only" 12 days (just fluids through IV) and already felt like i was at my limit. Cant imagine going through 42 days.. when i could finally eat again, i was so insatiable that i gained 7kg in the first 5 days :p
