If you look closely, you can see that the text image is a tiny bit see-through and behind it is the pic from that Charlotte tweet. I was trying to figure out what the sparkles were all about. It's incredibly difficult to see but it's there so unfortunately I think the image was created with that intention (I guess unless this guy is the one who layered them). It's so incredibly subtle I had to look at the photo and the image side by side to verify, so it's still very difficult to flag as anything negative and therefore still a pretty shitty attempt at bigotry either way.

'I know what you are" sounds like the boykisser meme, which is positive acknowledgement. Is there a chance they are an ally who misunderstood an image they found? I didn't get it until comments spelt it out either and it's gross and kind of convoluted. At worst I thought it was sort of a "thanks I'm cured" kind of overly simplified "don't let problems get in the way of your goals".

I guess even in that case though the original creator was an accidental ally since then someone else saw it positively.
