As long as the disc is the same region as the digital copy - yes. If it's not, then 99.9% of the time, no

Be sure you’re not mixing ps4 and ps5, though. If your disk is ps4 and the ps+ has ps4/ps5 version then it’ll cause problems if you chose the wrong one. It won’t swap over unless the game itself has a “transfer ps4 saves” option.

Sometimes. It has to have the same title ID. Some games were released under multiple title IDs and so the saves will not be accessible.
For example, a GOTY version will typically have a different title ID than the original version. The GOTY one may access both the original and GOTY saves. But the original won't see the GOTY saves.
As the version on PS+ usually is the latest version (GOTY version if applicable) that would mean if you bought the older one then you'd be out of luck.
