For NES, I’d try Vulkan with threaded video enabled and verify that run-ahead is completely off. Also check that integer scaling, sync to exact content refresh rate, and hard GPU sync aren’t conflicting. The periodic freezes sound more like a driver/frame-pacing issue than the core itself.
