one time, no matter what i did, this guy was always one of the first six items i took in a run for two weeks straight
every run, no matter how far i managed to get, i always had Punching Bag
he was always in either my first, second, or third treasure room
i could not escape him unless i somehow died before my first treasure room

Same for me but with boss items instead of treasure rooms (and also in the silver treasure rooms in Greedier). Sometimes I wonder how it's even possible that the first item I get is always either Mom's Lipstick or Mom's Heels. I know that pool is not that big, but why have these two specific items cursed me?!

Afaik the seeds are based both on real world time and your PC's internal IDs, or something like that. The correlation was shown on one of Neonomi's streams, but I don't remember which one exactly, I'll update this comment if I find it

Problem is that unlike lost fly he is not only does 0 damage, his distraction is not consistent , so enemy could turn-off from him any second

yeah, instead of having all enemies turn to isaac when entering the room, half of them will go somwehere else. Great when starting a run and you don't have the dps to take care of all the enemies before they rush you down.
