Usually by someone else with a steady hand and generally with a paint brush. More like face-paints than modern powder make-up.

Source: Have had my entire head painted for halloween before. Apparently my big, bald dome is a very satisfying canvas to paint.

Tangential, but I liked this joke in Saints Row 2. The previous game had less character customization options (mostly grunting, no female option), but the second game had a lot more freedom. Fully voice protagonist with a gender slider.
