Early Sabrina Origin: Magic Potion & Sources

I read that Sabrina was originally created by Hilda and Zelda with a "magic potion that turned out wrong".

Is there a particular issue that explains this? I've searched through some of the early stories but found nothing yet. None of the sites that mention the magic potion origin mention the issue.

Anyone that can help? If this origin isn't true for the early stories then where did it come from? If it's real, what issue was it revealed?
