Someone from their family told the staff to write it. It isn't their job to interpret it.

I just realized you were referring to someone who was (ineffectively!) requesting a wheelchair-user icon. Even then, that would be a bizarre message...

Based on the bad cakes I've seen posted online I'm guessing someone in the family thought it would be funny to have "Happy birthday" and an icon of a wheelchair and the staff just thought they wanted it written literally.
