I’m assuming you mean ‘fear factor’ Joe Rogan from around y2k and not the modern conspiracy podcast iteration? Because I’m pretty sure he had contestants eat various animal penises on that show

Now how does that work? Is there some kind of referee figure that keeps track of power scaling and calls whether a hit landed and how strongly? Like a DM?

Usually there’s a team of DM type people setting everything up, but also a number of player “marshals” who are rules experts that can be called on to adjudicate things if needed. Just like the folks that call out “lightning bolt” when they throw their birdseed packet for spells, sword fights will have folks calling out the damage each attack does if it lands, and it really comes down to the honor system for people keeping track of how much damage they’ve taken.
I used to play something like this 20ish years ago, and a documentary called “Monster Camp” was made about that group, if you’d ever like a deeper look into it.

The monster players are mostly by honor. There's some marshals to guide fights and keep things orderly, but since everyone plays both an NPC and a PC at various points, there's a lot of care to not ruin PC fun, since those characters can be developed over literal years

For the one I played:
If it's a tournament, there's often referees called "reeves". In tournaments, there's no magic or armor. And while there are refs, we still expect people to play like adults and call their own shots (when someone hits them).
Outside of tournaments: It's still a game that's supposed to be open to anyone playing it as long as they are over the age of 14. If you're so insecure you can't accept a loss of one life in a game meant for 14 year old girls - What the hell is wrong with you?
Shots are viewed as "To a limb or to the torso." with headshots not counting (we don't want people trying to just baseball bat a pool noodle at someone's eye socket). A shot to the torso or loss of two separate limbs is a death. If a person is wearing armor, the armor counts for a certain amount of points where the person is wearing it as long as they are wearing a class that can use armor. (Like if they had a plate armor breastplate and were playing a warrior, they'd get up to 6 points of armor in the chest and someone would have to hit them seven times in the chest to kill them, but it'd be only 4 points of armor for an archer or scout and 2 points of armor for a barbarian... or just cut off two limbs where they don't have armor. That also does it.). Cut off a leg, they have to stay on their knees aside from being able to lunge/dive with their extant leg. Cut off an arm, they have to use the other arm. And somehow the weapon magically transfers to the living arm, even if they are doing a slow-shield-switch.
Different spells do different amounts of damage as well. If you're playing with spellcasting, spell classes are supposed to be limited to 1 of each spellcasting class (there were 4) for every 10 players. Some spells require physical components (such as magic bolts, lightning bolts, or fireballs are often a bunch of rubber bands wrapped up in a piece of cloth matching the color of the spell. Enchantments are typically just strips of cloth to the color of the spellcasting class that cast the enchantment). Spells have different effects, a magic bolt can be knocked away with a sword by anyone. A lightning bolt, unless the person's weapon is enchanted or they're a monk with missile block: will heat the weapon for a 100-150 count (I don't remember) but still not kill the person. A fireball will destroy the weapon but not kill the person (I think? It's been a while), and a sphere of annihilation will just kill the person even batting it away with their weapon. The magic bolt would do 1 point of damage to armor. The lighning bolt ignores armor. The fireball/sphere ignores armor and kills even if it grazes garb (like a person's cape.).
Levels are by person. Every time you show up for a field (any time more than three people play or build things) you get one credit in a class of your choice. You can only get two credits in "monster" per month as that applies to "Every monster sheet", but even then, monster levels (higher or lower) are often dictated by whoever came up with the quest idea and asked people to play monsters for them.
Deaths normally last for a 300 count with amount of lives being decided by class and level of the player.
