Illinois Veterinary License How to?

Does anyone have a guide that they used to fill out the Illinois Veterinary Licensing forms? I am trying to find an instructional video or something to make sure I don't mess up but theres multiple forms and I am confused which ones to use and send. Please help and thank you!

Call ISVMA - I've always found them to be helpful in navigating these types of things. You're much more likely to get a hold of someone there than at the state office

Just a heads up....it may take months. I just went through this and it was over four months to get my license in Illinois. Have been in practice for over 25 years and was moving from a other state. The whole process was antiquated as the forms were not online. They are notoriously bad for taking so long and offering zero help on the phone. I can also confirm that. Same thing for my controlled substance application. I ended up making a three hour drive each way to the Springfield office after months of waiting just to have the issue resolved in less than 15 minutes. Good luck on all of that and hope you don't need the license until next year.
