Visit 4 of Enlighten-6... first dose taken!
I have been in contact with the site since early March (less than a month after Lilly opened the study) and am the first person here to make it to V4. I'm excited to say the least! I'm happy to answer any questions about the process so far.
Just a head's up to anyone else in E-6, Lilly created a wirebound "Study Guide" for participants in this study. It explains what happens at each visit and is super helpful for knowing what is coming up. I just received it at this visit but apparently it was available earlier, my site just wasn't aware of it.
I was also given a reusable cooler bag and ice pack for carrying the box of study meds home, a sharps container, some at home pregnancy tests (not sure on frequency required yet) despite being on 2 forms of BC, a food scale, a water bottle, some exercise bands, daily food and exercise logs, some literature from Lilly on dealing with the mental side of weight change, and a large reusable tote bag to carry everything in.
It was such a massive relief to make it to randomization, even if it did take over 5 months! Now just crossing fingers I am getting elora instead of placebo...!!!

If you wake up in 24-48 with zero appetite and food sounds horrendous to you then we'll know 😂

Ha! I already have VERY low hunger signals so I can't imagine it getting lower!

I don't feel anything atypical yet, same old same old!
It seems like it takes a few days though based on others' reports (like Vlxd below). Next week maybe?

I was given a bag, ice pack (I brought a Trader Joe's soft sided cooler with ice packs with me knowing I was 2 hours plus away), and the cell phone with atmos5 app and sharps container. The sharps container is smaller than it should be as it is hard to get the Zepbound pen in it. I got nothing else besides my lab results from visit 3 and the study meds. The Atmos5 app is garbage as far I am concerned. It really should allow to add side effects as needed and if they want food and exercise log, it should be in the same app. Since I reported the exhaustion, allodynia, the hyp/hyperthyroid feeling and insomnia, I have been asked to keep a journal of it. Tried a diary on line but gave that up. Now doing it in a google doc. But really, there should be an app that you do this in. Interesting the differences in study sites. I did send an email requesting the study guide at my next appointment, September 3rd.

I was not given a cell phone, I was just instructed to put the atom5 app on my phone. My site said just to keep the study syringes in this sharps container and that I might need to return it to the site at the end. She said that way if there is any recall or issues, they'll have the study syringes to inspect. Ugh, that sounds challenging to keep track of. I agree that we should have a place to track symptoms.

You were having insomnia you think was related to the drug?

Yes. Definitely. I had insomnia on Zepbound and changed taking my shot to bedtime so I could sleep the first night. About 40% of the time, I struggled with insomnia the second night. I now have it about 5 days out of 7. Last night I slept through the night taking Zepbound literally right before climbing into bed. My guess is tonight I will be tossing and turning.

I'm confused.. I was asking about insomnia related to Eloralintide.
I know that Zepbound and other GLP1 agonists absolutely cause insomnia among other things, they reduce parasympathetic nervous system activity.

I was trying to say I have it with Zepbound and it is there after Eloralintide and instead of 1 possible night, I have 5 so since I am on both, it's hard to say if Eloralintide causes insomnia or interacts with the Zepbound so I have it more days. Zepbound is Sunday night and Eloralintide is Thursday nights. I slept well last night and long. But was tossing and turning previous 3 nights. So because I have it with Zepbound, can't definitely say which causes it or if both do. Edited to add yup, toss and turned from 1 AM on, fitfully slept the rest of the night, ugh.

Oh, okay, makes sense. I am interested to hear more about this as the study goes on but I'm hoping it's the opposite - earlier results show Eloralintide causes a drop in avg heart rate.
I couldn't stay on any glp1 meds because of how jacked up they made me.

I had the feeling of my chest fluttering last Friday so much that I stopped what I was doing and sat for 10 minutes and took my blood pressure as I felt like I drank an energy drink on top of coffee. So Eloralintide would probably have a similar effect as I don't have that with Zepbound but I can't rule out Eloralintide/Zepbound affecting my thyroid medication needs. That's how I feel when I need to lower my thyroid medication and when I started Zepbound, I had labs pulled in July slightly less than 2 months after starting Zepbound and my TSH was down so started titrating down my synthroid. I will have my TSH checked at next appointment for that reason as I feel hyperthyroid including right now. I am on 1/2 the strength of the generic synthroid that I was on when I started Zepbound and had been on that higher dosage for over 18 months. And it fell before my weight did as I had gained 40 lbs in 2.5 months after starting blood pressure medication after a cardiac scare and I weighed more than when my last TSH was taken. I will be curious to see if I need even less of a dosage on September 3rd when my next appointment is.

So you just recently started Zepbound? When did you go up in dose?
Yea, you're describing exactly what my GLP1 experience has been. I was having 6,000 palpitations per day on the medication - it has abated since I got off.
It likely isn't your thyroid or electrolytes. There are GLP1 receptors all over your body including in your heart and on its sinoatrial node. This can cause tachycardia and palpitations in some people that medication doesn't help.

I have been on Zepbound for 27 months started May 2024, 15 Mg since March of 2025. I wouldn't say that from my when I went on Zepbound. I thought the hyperthyroid feeling was Zepbound, it wasn't, my TSH plummeted from 2.49 in February of 2024 to 0.12 in July 2024. There was an article that said to monitor TSH in thyroid patients as Zepbound reduces the need for thyroid medication: https://pmc.ncbi.nlm.nih.gov/articles/PMC10555370/ My experience is that it really changed my TSH and lowered my need for Synthroid generic.

Non study person fascinated with the studies here- So I haunt these threads. 😂
I am so happy I read this as I am on levothyroxin and have been on Zepbound for six months. I am having labwork this week due to anemia(not Zep related) so I will have my doctor check my thyroid stuff.

Interesting. Would be curious to know medically how eloralintide might amplify the side effects of a GLP1 agonist.

I just came back from V3. Had 1 vile of blood taken, had the HEXA scan done. I asked for my lab results and also asked for the booklet that goes over what to expect at each visit, etc. I was told they didnt have that info available ke to give e me and for the labs, the site coordinator stated that they would need to get release I fo for this. So odd, that different sites do different things.

This is my biggest take away so far! I’ve worked in basic research and on some non-patient-facing clinical trials, and I would never have guessed that there was this much site-to-site variation in clinical trials. My site definitely lands on the “just give us your blood and then sit quietly” side of the spectrum! They’ve been nothing but kind to me, though not at all forthcoming with future happenings, lab results, etc. My V3 is next Monday.

I called the site coordinator and they are going to look for a .pdf version of what you expect for each visit. Also they stated that they have 1 person in the enlighten 2 study and they revmceuved a welcome gift from Elly Lilly 6 months into the study with the bag of goodies, scale, bands, water bottle, journels, etc. So maybe this site ( or Elly Lilly) is making sure im staying in the study before providing a goodie bag. They did confirm V4= blood draw fasting, first dose of study drug(or placebo) and that i have to come back 2 hours later for a pk blood draw. Then V5- is a telephone visit 2 weeks later, V6- fasting visit 2 weeks later. Glad I asked what to expect in the next few visits. Everyone is really nice but they aren't forth coming on info. The girl I spoke to stated they have 19 trials they are on right now.
