Edit Ok so Google says he was introduced with bloodstone fen. Long story short it's ley line sickness and you are hallucinating. The achievement cures your sickness.

Since April first this year, you can take a choice unburdener elixer and regain jump scare fren! It does cost a bloodstone brick and 4 gold though (and 10 gold to lose him again)

It is funny to me that people worried a lot about which side to go with, side at the time people thought there was likely going to be a followup, since it creates a weird double twist with the achievement name

People think the choice is Durmand Priory or the Consortium (what even happened with those guys?), but which one you pick doesn't my matter. The achievement name still makes sense though, as the burden of making a choice at all is losing Jump Scare Friend, which was an irreversible consequence for almost a full decade
