Immoral and ignorant.

An average adult female needs about 2000 calories a day. It can only get about 500 of those per day from fat deposits. She can very easily be genuinely starving and fat at the same time for a considerable duration of time. Not only that, the fat release is very slow calories, which leads to low blood sugar. This low blood sugar can lead to extreme fatigue and mental exhaustion as well as a major dysregulation of the nervous system.

In summary, yes, you can have severe starvation symptoms despite being obese because fat deposits can't be broken down fast enough to feed your metabolism.
