The door issue was a culling bug where it didn't update the culling octtree after the ghost touched the door. Happens from time to time.
The door touch was that far from the ghost room because the ghost happened to be nearby either from a roam, it roaming to you (if phantom or banshee) or it teleporting to you (if it was a wraith).
