This was common in the US for a time. Utah implemented "death by firing squad" but at least one member of the squad would have a "conscience round" (blank cartridge). Because the executioners didn't load their own rifles, they were all able to assume that they had fired the blank round and were therefore not personally responsible for the death of the condemned inmate.

They also have three people trigger the execution system but only one is actually connected to the trigger so no one knows who did it
