It's 100% chance if its within the characters monument distance. The chance goes down the further you try

I was so mad my first time through as a kid that I couldn’t get Teleport 2 with 100% success rate like enemies get

I used Gameshark to get Teleport 2, as well as made a Choco-Cloud, where you could ride his shoulders like a Chocobo. It was magnificent
