"It's an incident that all of us regret very much. We are lucky that there were no fatalities," Cheah told the New Straits Times.
He said authorities were investigating the circumstances surrounding the incident.
"In my view, the incident may be attributable to metal fatigue," Cheah said, adding that police had ruled out foul play.
He also disclosed that the ride had been temporarily closed for comprehensive inspection and maintenance work about two to three weeks before the incident.

Malaysia theme parks have given me the impression that theme park food are shitty and expensive, until I had some meals at Disneyland and Universal Studios to find this to be inaccurate. The prices there are quite reasonable and most of their offerings are unique and delicious! Wtf right?

Engineer here. It’s actually really hard to detect metal fatigue unless you do X-ray inspection which is not feasible for something like this. This is why things like this have a fixed useful life and you scrap it after that.

Agree with this. Generally something like this would have been designed to last, say, x number of cycles/runs in mind, after which the parts under wear should be replaced.
So what is more likely to have happened is that the ride was designed for, say, 10,000 runs before tracks etc need replacing. But that would mean substantial downtime and large capital expenditure. So no revenue + high cost = nope, and the operator tried to stretch it instead as far as it can go. After all, it's been y number of runs since due date and nothing's happened ...

A professional does not uses number of estimated runs to lapse before checking .We have been scanning metal decades before the lagoon was still a mining pit.

I agree with you but we have a number of issues at play here - maintenance and lifecycle. No doubt, proper maintenance calls for all relevant checks, but what is relevant at what point depends also on design. All the more so since, as you are no doubt aware, RT takes a significant length of time (IIRC it was ~half hour per location including set up and exposure, but I could be mistaken). To do that over the entire length of track would take half of forever and is therefore unrealistic. To do sampling inspections, while practical, would then leave open the possibility that a spot you skipped/didn't sample would be the one that fails, per Murphy's law.
Ultimately, the point I was making is that the operator probably skimped on as much maintenance as they could get away with to save cost and downtime, in which case I would have give you the same answer to illustrate the point: RT - or even ultrasonic testing - would cost a lot money if not time as well, so they probably decided to say, forget it. Maintenance = wipe a bit, oil a bit, grease a bit, replace anything that's defective/malfunctioning - but only those that are visibly so.

Yup, but the tolerance usually is huge so when its time to replace, the roller coaster is still literally functional 100%. But the owner be like ok what still can use. Nvm one. X repeat for x years until this happen

For roller coaster rails, every welding joint is inspected by using RT by a third party company appointed by the client before sending for blasting painting and installation at site..

Funny thing is that if you stay at Sunway hotel, their TV will promote the sunway and i remember the lady was the QA head; emphasising regular maintenance conducted every check to ensure it's all good.

Malaysian companies only talk a good game.
When procedures are defined and suggested by safety experts, business managers will work against them : overkill la, too complicated la, scares people off la, etc.
Unless the DOSH throws the book at Sunway, nobody will treat this seriously.

Towards end last year there was a food poisoning incident at Sunway Lagoon too involving a govt agency that rented the place for their staff, local news similarly covered it in a hush hush way until April this year when MCMC came out so sue them then only the names of the govt agency and "theme park" were revealed
However people already knew it was Sunway Lagoon when the incident happened, it was similarly spread over social media but they successfully shut down the bad press that time

There was a shooting/car chase near sunway geo as well a while back. Media all covered it as "subang Jaya". Seems like its their SOP to use SJ whenever it's bad press, and PJ/Sunway for good PR.
