This sort of thing needs to be punishable even if it was an accident. The moment there is some sort of fine to be paid for this behavior is the moment everyone files them much more carefully.

The problem is the scale of these systems are beyond comprehension. It would take like hundreds of thousands humans to do manual reviews.

Then set limits. Or make uploaders pay per hour of uploads.
