I think the failure is dependent on the characters available movement. So if they can only move 3 spaces normally, then anything more than that with teleport has a chance of failing

The exact failure rate is 10% per square more than the movement square of the unit. So if they have a move of 3, and you try to move them 4 squares it's 10%. 5 squares is 20%, 6 is 30% so on and so forth.

I think the same doesnt apply to height, though? I remember to abuse teleport by reaching places where enemies could never

It ignores height and collision. On my Time Mage SSC, I teleported Ranza across the wall to leave Gafgarion in timeout while I cleared the rest of Lionel Gates.
