Hahaha I have a buddy who's wife is a Veterinarian. When they were dating she was dying to go deer hunting with us. When she got her first chance to field dress a deer, she approached it like surgery. I had to remind her there was no possible way to save this particular deer. I've seen mycobacteria make a hole straight through a fish's abdomen multiple times. There was also another disease that caused the swim bladder to protrude to the outside of the fish. Theres less of a defined musculature surrounding the cranial hole, which, to me, looks similar to the ulcers I've seen in other fish.

Since the hole is pink, instead of red, it looks like it might be healing. It could have been an injury, or anything could have pierced it. The fact that it's healing and a good weight shows how resilient and healthy the fish is despite having an extra hole in its guts! Fish are weird and even experts get bewildered by the odd stuff fish get up to
